This announcement solicits applications for the Primary Care Services Resource Coordination and Development program. The Primary Care Services Resource Coordination and Development Program is intended to facilitate the coordination of activities within a state that relate to the delivery of primary care services and the recruitment and retention of critical health care providers. This includes working with other agencies within the state government, as well as organizations outside of the state government whose policies affect health care services. This program does not support the direct delivery of services. The purpose of this funding opportunity is to support states and territories in addressing the following overarching goals: Statewide Primary Care Needs Assessment Shortage Designation Coordination Technical Assistance and Collaboration that Seeks to Expand Access to Primary Care
